Program Eligibility

If you are currently using a non-Tandem Diabetes Care insulin pump for your insulin therapy and are interested in upgrading to the t:slim X2 insulin pump with Basal-IQ technology or Control-IQ technology, you have the option to use a new t:slim X2 pump for the remainder of your current pump warranty for a one-time payment of $999.* This pump upgrade opportunity expires December 31, 2022.

Terms and Conditions

Participants in the t:slim X2 Insulin Pump Upgrade Program (“Program”) must be able to cover the costs of supplies for the duration of their original pump’s existing warranty (“Program Period”), either through their current insurance plan or other standard payment options. Purchase of supplies directly from Tandem is not required; in most cases participants can continue to purchase supplies from their current distributor. Insurance verification, including confirmation of duration of the participant’s current pump warranty, and a prescription will be required to start the Program. Payment plans are not available.

At the end of the Program Period, the t:slim X2 insulin pump must be returned to Tandem. Failure to return the pump within thirty (30) days following the end of the Program Period will result in a charge for the full cash price of the pump, less the prior $999 one-time payment.

Program terms and conditions are subject to change at any time, without notice. Limitations apply. This offer is not available for people under 6 years of age. Offer is not valid for: any person insured or subject to reimbursement in any part by Medicaid, Medicare, Tricare, or any other federal or state health programs (such as medical assistance programs); members of a Kaiser Permanente Health Plan; members of health plans or insurance contracts that prohibits this Program; residents of any state where prohibited by law and persons residing outside the United States and Puerto Rico; or where otherwise prohibited.

The Program is available only for current non-Tandem pump users who have at least six months remaining on the warranty of their current pump. Offer not available to individuals with in-warranty pumps who are eligible for a new pump through their insurance plan. The remaining warranty duration of participant’s existing pump will apply to the Tandem pump received through the Program. Warranty support for the Tandem pump during the Program Period shall be in accordance with the warranty accompanying the product. This offer will expire December 31, 2022.

Important Safety Information

RX ONLY. The t:slim X2 insulin pump, Basal-IQ technology, and Control-IQ technology are intended for single patient use. The t:slim X2 pump, Basal-IQ technology, and Control-IQ technology are indicated for use with U-100 insulin only.

t:slim X2 insulin pump: The t:slim X2 insulin pump with interoperable technology is an alternate controller enabled (ACE) pump that is intended for the subcutaneous delivery of insulin, at set and variable rates, for the management of diabetes mellitus in people requiring insulin. The pump is able to reliably and securely communicate with compatible, digitally connected devices, including automated insulin dosing software, to receive, execute, and confirm commands from these devices. The pump is indicated for use in individuals six years of age and greater. Basal-IQ technology: Basal-IQ technology is intended for use with a compatible integrated continuous glucose monitor (iCGM, sold separately) and ACE pump to automatically suspend delivery of insulin based on iCGM readings and predicted glucose values. The bolus calculator is indicated for the management of diabetes by people with diabetes by calculating an insulin dose or carbohydrate intake based on user entered data. Basal-IQ technology is intended for the management of diabetes mellitus in persons six years of age and greater. Control-IQ technology: Control-IQ technology is intended for use with a compatible iCGM (sold separately) and ACE pump to automatically increase, decrease, and suspend delivery of basal insulin based on iCGM readings and predicted glucose values. It can also deliver correction boluses when the glucose value is predicted to exceed a predefined threshold. Control-IQ technology is intended for the management of Type 1 diabetes mellitus in persons six years of age and greater.

Warning: Control-IQ technology should not be used by anyone under the age of six years old. It should also not be used in patients who require less than 10 units of insulin per day or who weigh less than 55 pounds.

Control-IQ technology and Basal-IQ technology are not indicated for use in pregnant women, people on dialysis, or critically ill patients. Do not use Control-IQ technology if using hydroxyurea. Users of the t:slim X2 pump, Basal-IQ technology, and Control-IQ technology must: be able and willing to use the insulin pump, CGM, and all other system components in accordance with their respective instructions for use; test blood glucose levels as recommended by their healthcare provider; demonstrate adequate carb-counting skills; maintain sufficient diabetes self-care skills; see healthcare provider(s) regularly; and have adequate vision and/or hearing to recognize all functions of the pump, including alerts, alarms, and reminders.
